To apply for IHSS yourself or on behalf of an applicant, you will need to have the following information available:
• The correct spelling of the applicant’s first and last name
• The applicant's date of birth
• The applicant's Social Security Number
• The applicant's address (both residence and mailing, if applicable)
• The names of other people living in the residence with the applicant and their ages.
